Tanning as a Minor *

I understand that if I am a minor, I have to bring a signed permission form from a parent to tan and will not be eligible for 24 hour tanning. No one under the age of 14 allowed to tan period. Minors can tan during business hours only (10 am - 6pm) and only with parental permission.

24 Hour Tanning *

I understand that cameras are in use for security reasons after hours, but no cameras will be in the tanning area. If I am a minor, I agree to only tan between the hours of 10 am and 6 pm. I promise not to leave a mess, destroy property or do anything other than use the tanning rooms and Awaken property for any other reason other than tanning when I use the 24 hour privileges. I understand any negligence to adhere to these rules will end in suspension or termination of my tanning membership without a refund.
